Once an outpatient provider match is identified, your referral counselor contacts you with the provider’s name, credential, location, and phone number so you are able to schedule an appointment directly with the provider.

Your resource and referral counselor will follow up with you to ensure the match meets your needs. If you need additional support in the future for other referrals or if the match does not work out for some reason, please know that you can always call the Helpline again.
